Master's Forms
MSCS Degree Plan (pdf)
Usage: The Degree Plan is used to map out an intended sequence of courses leading to the MSCS degree. The form will normally be completed during initial advisement, and may be updated during subsequent semesters. The form is also used to double check that degree requirements have been met.
Due: Initially, by the end of the first semester of enrollment. Ultimately, no later than submission of the degree petition.
Special Problems (CS 8903) Permit Form (pdf)
Usage: This form allows a student to register for a specified 8903 section.
Due: By the end of Phase I registration.
Master's Project (CS 8902) Permit Form (pdf)
Usage: Prior to registering for CS 8902 (Master's Project), the student must complete this
form and a brief description of the proposed project.
Due: One week prior to the beginning of Phase I registration.
InfoSec Practicum (CS 8902) Permit Form (pdf)
Usage: Prior to registering for CS 8902 INF (InfoSec Practicum), the student must complete this
form and a brief description of the proposed project.
Due: One week prior to the beginning of Phase I registration.
MS Thesis (CS 7000) Permit Form (pdf)
Usage: For approval to register for MS Thesis Hours.
Due: One week prior to the beginning of Phase I registration.

PhD Breadth Component Form (pdf) (HTML)
Usage: For approval to register for PhD Thesis Hours. Must be a degree candidate.
Due: One week prior to the beginning of Phase I registration.
PhD Minor Form (HTML)
Usage: For approval to register for PhD Thesis Hours. Must be a degree candidate.
Due: One week prior to the beginning of Phase I registration.
PhD Academic Advisor(HTML)
Usage: For designating or changing advisors.
Due: Upon selection or modification of Advisor.
PhD Qualifier Form (pdf)(HTML)
Usage: For approval to register for PhD Thesis Hours. Must be a degree candidate.
Due: One week prior to the beginning of Phase I registration.
PhD Thesis (CS 9000) Permit Form (pdf)
Usage: For approval to register for PhD Thesis Hours. Must be a degree candidate.
Due: One week prior to the beginning of Phase I registration.
Dissertation Research Defense Form (HTML)
Usage: To document the dissertation defense and any conditions for successful completion.
Due: This form should be submitted to the Ph.D. Program Coordinator immediately
after the dissertation defense.

Usage: This form allows a student to register for a specified 8903 section.
Due: By the end of Phase I registration.
Graduate Student Exit Survey (pdf)
Usage: To be completed by all graduate students in conjunction with the petition to graduate.
Due: Prior to the deadline for graduation petitions.
Request for Transfer of Graduate Credit for CS Courses (pdf)
Usage: This form is used to request graduate level credit for courses taken at other institutions. Credit can be used to reduce the course requirements or to meet curriculum requirements, subject to the limits of the Institute and specific program.
Due: Typically no later than the end of the first semester of enrollment.
Undergraduate Request to Register for Graduate Course (pdf)
Usage: This form is used by undergraduate student who are attempting to register for graduate sections.
Due: Prior to the end of Phase I Registration.
Student Request for Travel Funds (pdf)
Usage: This form is used to request reimbursement of travel expenses associated with attending a
conference, workshop or other sponsored activity. Submission of this form does not ensure a student will receive funding. Limited funds are available for such purposes and students should request funding as soon as the need is known (acceptance of a paper, etc.).
Due: Normally, prior to making reservations for the trip; no later than one week prior.
